(MANCHESTER, Ind.) – Manchester Township Fire & Rescue is the first agency in Dearborn County to launch a Mobile Integrated Health (MIH) program.
The program will be provided in a patient’s home at no cost and the goal is to reduce unnecessary emergency room visits and improve patient outcomes.
“This program is designed to bridge the gap between pre-hospital care, emergency departments, and primary care providers by offering proactive, in-home medical support focused on prevention, education, and long-term patient outcomes,” stated the department.
Through this program, Manchester Township Fire & Rescue will provide services including chronic disease management, post-discharge and post-refusal follow-up visits, prescription and medication management, and general health assessments.
Providers will also assist with identifying fall risks, evaluating living conditions, and helping patients navigate available healthcare and community resources.
“Manchester Township Fire & Rescue will work closely with the Dearborn County Health Department to ensure patients have access to additional resources. This includes providing fall prevention kits, educational materials, and connecting patients with services such as mental health support, substance use resources, and other community-based programs,” the department said.
As the program is implemented, services will initially focus on Manchester Township residents as part of a pilot phase.
"This approach will allow the department to evaluate effectiveness, refine services, and build a strong foundation for future expansion throughout Dearborn County," added the department.
The MIH program will begin in June. Follow their Facebook page for more updates.
